Chronic kidney disease affects 30-50% of cats over age 15. While CKD cannot be cured, proper nutrition is the single most impactful intervention โ€” renal diets can extend survival by 2-3 times compared to standard diets.

Why Renal Diets Work

Prescription kidney diets (Hill's k/d, Royal Canin Renal Support, Purina NF) are formulated with reduced phosphorus, controlled high-quality protein, added omega-3 fatty acids, potassium supplementation, and B vitamins. These address the metabolic consequences of kidney failure.

Reducing Phosphorus

Phosphorus restriction is the most critical component. High phosphorus accelerates kidney damage. Renal diets limit phosphorus. Phosphate binders (aluminum hydroxide) are added to food when diet alone is insufficient. They bind dietary phosphorus in the gut so it is excreted rather than absorbed.

Protein: Quality Over Quantity

Contrary to old beliefs, protein restriction is NOT the primary goal. Controlled amounts of high-quality protein maintain muscle mass while minimizing uremic toxin production. Cats need protein โ€” muscle wasting is one of the biggest quality-of-life issues in CKD cats.

Increasing Water Intake

CKD cats cannot concentrate urine and lose water. Counter this with canned food (70-80% moisture vs 10% for dry), multiple water bowls and pet fountains, adding water to food, and subcutaneous fluids at home for later-stage cats.

What If Your Cat Won't Eat Renal Food

A cat that won't eat is more dangerous. Try warming the food slightly, offering different brands and textures, transitioning gradually over 7-14 days, using appetite stimulants (mirtazapine), and as a last resort, any food is better than no food.

Monitoring at Home

Track weight weekly (kitchen scale), appetite daily, water intake, and litter box habits. These catch problems weeks before blood work does.
